Thank you so much for such a great year!  The 2011-2012 school year is coming to an end.  Here are some important dates regarding the semester schedule:

 

Spring semester - Ends Sunday, June 17

Summer semester - Monday, June 25 to Friday, August 31

Fall semester - Begins Tuesday, September 4

 

Please let us know about your summer and fall plans by filling out the following form.  If you are discontinuing or is unsure, please also fill out the form to notify us.  Be sure to hit the "Submit" button at the bottom upon completion of the form.  Please submit the form before 6/2 to help us co-ordinate scheduling efficiently.  You may fill out one form for the whole family if the schedule and all the information is the same.

Please notice there is a "Your Feedback" box at the very bottom.  We would love for you to share with us any thoughts or comments that could help us provide you with better service in the future.  If you have feedback but wish to do so anonymously, you may fill out an extra form (after notifying us of your plans) and only fill out the feedback portion (remember to hit the "Submit" button).
