Spring
Monday, January 29, 2024 to Sunday, June 16, 2024

Summer
Monday, June 24, 2024 to Friday, August 30, 2024

 

There is a $35 per family registration fee to register for each school year.  The fee will be added to your first invoice of the school year.

For families with multiple students, you may fill out one form if lesson availability is the same.

Please write down in the Additional Notes box at the end of the form which instrument(s) is for which student(s) if there are multiple students taking different instruments on the same form.

Lesson Availability

Please let us know about your availability during the school year by filling out the following. Please be certain of your schedule before you let us know because it's very difficult to change it after it's scheduled since it involves moving many others.

Please fill in the earliest possible start time and the latest possible end time on the days available for lessons. Please try to give more than one day and time if possible, as there are many factors in considering a schedule, especially a teacher's driving route.

Payment Options

Your monthly balance will be settled by automatc payment through either credit card or ACH (fund transfer from your bank account). By selectng your preferred payment method and entering corresponding the payment information below, you hereby authorize Merry Melody Music Academy to automatically debit your credit card or bank account listed below on the FIFTEENTH (15th) of each month for the amount owed to Merry Melody Music Academy, Inc. asdetailed in the monthly invoice received by the EIGHTH (8th) of each month.

You agree to notify Merry Melody Music Academy in writing of any changes in your account information or termination of this authorization by the 1st of the month. You understand that cancellations must be made in writing and you will not dispute Merry Melody Music Academy's automatic payment with your bank so long as the amount corresponds to terms indicated in this contract.

You guarantee and warrant that you are the legal cardholder/duly authorized check signer on the below account(s), and that you are legally authorized to enter into this automatic payment agreement with Merry Melody Music Academy.

If you pay by Credit Card, there will be a 2.9% convenience fee added to each invoice.

If you pay by ACH, there is no fee.

The invoices will include Registration Fee, Lessons Fees, Books and Equipment Charges, and Recital Fees, as detailed in the School Policies.
